I got a email that appears my submission form is missing and a contact number. Obviously being after hours it's visibly not a good time.
Has anyone faxed or emailed a updated submission form? Any help would be appreciated.
I don't know how they knew my email but I'm glad they contacted me.

Never had this happen but I'm sure they got your name off the package and cross checked it for your account. Hopefully you kept a copy (I usually scan mine with the cards) and can send it to them. Im sure its mostly for special requests that they need it but I would send asap to avoid delays.
