5. Must feature LT only on the card (no dual autos, etc.)

I have a 2009 Bowman sterling refractor auto and it fits all of your rules. I would want Beckett value though. I can post scans if you're interested

Thats a lot of rules, I have one that is /2 that would fit. Can provide a scan if you wish, but it will be pricey...
