In addition, Upper Deck offered dealers a 5-card Predictor pack with the purchase of one case (20 boxes) of first series product. Dealers were given all 20 first series Predictor cards (retail Player of the Month and hobby Player of the Week) with the purchase of two cases. Each card lists months that the featured player might win Player of the Month honors. Black and red basketball court fronts frame a full-color action player cutout. A black border surrounds the player's name, team and the month of the predicted award, all of which are stamped in gold foil. The outer border of the front is a black marble texture. Numbered backs are printed on white, have the prefix "R" and explain the rules of the game. Those holding a winning Predictor card redeemed the cards through a mail-in offer for a full set of the Predictor Player of the Month cards.
