online submission form - nismo68 - 04-01-2013
Trying to fill out a form online and when I get to "card key:" I get lost. What info is that? This wasn't on the old form before. Any help is appreciated.

I just start labeling them 1, 2, 3 etc, etc. Makes no difference what the number is that just needs a number inserted.
