I have never done it, but I think you have to just fill out the form, and pay with a credit card so they charge your twice. Once with the review fees, then later for any additional for the crack fee and grading fee.

im not sure if i understand the explaining of the
crossing over minimum grade. if i submit a PSA 9 card in a case and say i would like a
minimum grade of 9, if it is reviewed and will get a 9, they will still crack it
open and put it into a BGS case with a 9 right?? and if it was to get an 8.5
instead, then they will leave it in the PSA?
I want the cards in BGS cases, but only if they will stay
the same grade..

This is correct.. You must specify your minimum grade if BGS feels the card in question won't cross to that specified grade they send it back in the holder it came in...
What kinda stuff you looking to cross? maybe you could post a few scans (if possible) and get some insight..
In my experience, I've never seen a psa 9 get a BGS 9; it gets a BGS 8.5 or BGS 8. A psa 10 gets a BGS 9.5 or even a BGS 9. psa is really lenient on centering and surface.

I have not had the best of luck with crossovers myself... surface is usually the culprit as I've always tried to cross centered stuff...

btw, they are 1986 fleer basketball cards.. i would like to sell the set oneday as an all graded 9 set and figured if they were all or mostly BGS it would increase the value somewhat?
there just havent been many BGS 9's popping up lately to complete it... mostly PSA
i do try to get the best centered/corner/edges ones, but cant really tell about the surfaces online...

2. For pricing, please calculate $10 per card for the review fee ($7.50 per card if submitting 4 or more cards).
3. Select the service level (turnaround time). When your cards are reviewed, any cards crossing over will be graded and processed under the turnaround time selected. Your credit card will be charged for the selected service level, plus a $3 cracking fee per card. You MUST pay for the GCR service with a credit card. The reason for this is that if the minimum grade is reached, you will be charged additional fees for case removal and the standard grading fee. An invoice will be mailed back with the order to show you the exact fees you were charged. If the minimum grade is not reached, you will only be charged the review fee.
